Healthcare Requirements 

Middle School & High School

 

To keep your children safe and healthy, and according to the State of NJ and Dept. of Health & Education, please note the following Healthcare requirements MANDATED BY LAW, as all students must have up-to-date ANNUAL (yearly) health information on file at school.  PLEASE fill out the required forms, and return all paperwork to the School Nurse.  Forms are located on the school website for your convenience.  Thank you!

 

Allergy Action Plan form: 

If student has Allergies that might require allergy medication such as Benadryl or EpiPen during school hours/affiliations, have Healthcare Provider (HCP) fill out this form so the school can assist your child should he/she have a life-threatening allergic reaction.  Last years’ form has expired, and this form must be renewed yearly!

 

Asthma Action Plan form:

If student has Asthma and might require medication such as Albuterol during school hours/affiliations have HCP fill out this form, checking self-carry box at bottom, if appropriate, so the school can assist your child should he/she have an asthmatic episode.  Last years’ form has expired, and this form must be renewed yearly!

 

Diabetic Action Plan (Specific orders from Healthcare Provider - HCP):

If student has Diabetes have HCP write a Diabetic Action Plan so the school can assist your child during school hours/affiliations, and please send in ALL diabetic supplies for your child.  Your HCP can use their form.  Last years’ form has expired, and this form must be renewed yearly!

 

Health History form:  This form includes emergency contact information, parental/guardian permission to administer Advil or Tylenol, medical updates, insurance information, and helps the School Nurse to understand your child & how best to meet their needs, while keeping them in school.  Last years’ form has expired, and this form must be renewed yearly!

 

Immunization Record: 

Immunizations must be up-to-date based on age.  6th graders who are turning 11 years old require the Tdap & Menactra shots, and the Flu shot is highly recommended.  See specific Immunization requirements on the school website.  If you do not find an answer to your quesiton, and need further assistance regarding vaccinations, call the School Nurse.

 

Injuries:

If student is seen at the urgent care, ER or HCP facility for an injury, please request documentation indicating dates student is to be out of gym & sports school/affiliations.  Sometimes the HCP will write “follow-up with Primary HCP or Orthopedic HCP for clearance.”  In this case, the School Nurse will need documentation for clearance.

 

Medication Consent form:  Besides allergies, asthma, diabetes and seziures, this is the form to use for all other medications.  For example:  If student requires ADHD medication, this is the form to use.  Last years’ form has expired, and this form must be renewed yearly!

 

Medication PICK-UP END-OF-YEAR: 

At the end of the school year, all medication must be picked up and signed out by a Parent/Guardian during the last three days of school.  No medication will be given to students to take home, the school will not store medicines over the summer months, and all medication will be disposed of unless specific plans are made ahead of time between the School Nurse and Parent/Guardian. 

 

Seizure Action Plan form: 

If student has Seizures and requires medication during school hours/affiliations have HCP fill out this form so the school can assist your child should he/she have a seizure.  Last years’ form has expired, and this form must be renewed yearly!

 

Sports Physical Examination STATE form: 

Any student playing sports must submit the STATE sports physical examination form; no other form will be accepted for sports!  Grades K, 5, 7 or 8 and 10 require physical examinations.  If your child is not playing sports you can use your HCP Physical Examination form, OR State Sports Physical Examination form.  Last years’ Sports Physical Examination form has expired, and this form must be renewed yearly!

 

When to Keep Your Children Home from School: 

Clayton Middle & High School work with the GCDOH to protect children from infectious diseases.  Keeping children home when they are too sick for school protects other students and staff from potential illness.  If a student becomes ill at school, it will be necessary to pick him/her up as soon as possible.  If your child is sent home from school for an illness, please follow the guidelines below for returning to school.  If student has the following symptoms, please keep him/her home from school. 

 

  • Fever – Temperature 100.2 degrees Fahrenheit or higher.  Before returning to school, the student must be fever-free for 48 hours without fever-reducing medications.
  • Vomiting – Before returning to school, the student must be vomit-free for 24 hours after the last episode of vomiting.
  • Diarrhea – Before returning to school, the student must be diarrhea-free for 24 hours after the last episode of diarrhea.
  • Conjunctivitis (pink eye) - If a student has thick mucus or pus draining from one or both eyes, a HCP must treat your child then he/she may return to school 24 hours after treatment has begun, providing the school with documentation of clearance to return to school.
  • Lice – If student has live lice, he/she must be treated & may return to school after being free of live lice, after initial treatment.  The School Nurse will check student to determine if the student is free of live lice.

Working Papers:
