If your child needs daily/routine medication, healthcare provider must fill out this Medication Administration Form, sign & stamp.
The medication administration form should be used for daily/routine medication administration such as ADHD and/or migraine medications, etc., only.
Students are not allowed to carry any type of medication in school, except for inhaler or epipen if prescribed by Healthcare Provider to self-carry, with the documentation on file in the school nurses office.
All other medications must be brought to school by an adult who is at least 18 years-old in the original container/box with label indicating students name, etc.
Any medication that needs to go home at the end of the day must be picked up by an adult who is at least 18 years-old.
