Vision Screening Programs 

 

 

School nurses are involved in front-line vision screening to determine if students have vision problems and to facilitate follow-up care to ensure academic success within the classroom.  

 

If your child has frequent headaches is complaining of blurred vision or has difficulty seeing the board in class, please let your school nurse know so that she can assess your child and provide care as needed.  Good vision can help your child to succeed in school.  Seeing clearly is important for learning and even for playing sports and athletics!

 

 

VSP Global - Eyes of Hope Sight for Students Gift Certificate

 *If you do not have insurance or medicaid, this one is for you!

 

Covers the following at no cost to you:

  • Comprehensive eye exam
  • Otis & Piper brand frame up to $115.00
  • Single-vision, lined bifocal or lined trifocal prescription lenses in polycarbonate
  • Vision therapy may be covered if prescribed.
  • Lens enhancements may be available at an additional cost.  Discuss these options with your Healthcare Provider.

 

Important Information: 

  • May use one VSP Eyes of Hope gift certificate every 12 months.
  • Must be used before the expiration date listed on the front.
  • Your child must receive an eye exam and glasses from the same eye doctor’s office.
  • Lost, stolen or broken glasses received through the gift certificate will not be covered or replaced.

 

Instructions for using Gift Certificate: 

  • Find a participating VSP network doctor by calling VSP or visiting VSP website.  Translation services are available.Call the VSP network doctor you’ve chosen and make an appointment.  Let them know you have a VSP Eyes of Hope gift certificate for an exam, lenses & Otis Piper frame.
    • Visit vsp.com.
    • Select Find a Doctor
    • Enter your ZIP code and select View Filters on the right
    • Under Doctor Network, select Choice
    • Under Frame Brands, select Otis & Piper
    • Under Language, select your preference
  • Take the gift certificate with you to the appointment.

20/20 - New Eyes for those in Need

 

New Eyes purchases new prescription eyeglasses for low-income individuals in the U.S. through a voucher program. To be eligible for our program, applicants must:

  • Have household income at or below 200% of the U.S. Poverty guidelines.
  • Have had a recent eye exam with a pupillary distance measurement (or click here to learn how to measure pupillary distance) . New Eyes does not pay for eye exams. Contact Us if you need assistance in locating a source of free or low-cost eye exams. 
  • Have no other resources available to them to pay for glasses, including federal or state programs or assistance from local charitable organizations.

A New Eyes voucher provides a basic pair of single or lined bifocal eyeglasses.
