Healthcare Requirements

 

 

To keep children safe and healthy, and to give them the best possible care during school hours/activities, fill out the Healthcare Forms that apply to your child, including the Over-the-Counter Consent Form, and return to the School Nurse at the start of the school year.

Healthcare Forms:

Required and Recommended:

 

Allergy Action Plan/Medication Consent Form

Required if student might need Emergency Allergy Medication during school hours/activities

Asthma Action Plan/Medication Consent Form

Required if student might need Emergency Asthma Medication during school hours/activities

Diabetic Action Plan/Medication Consent Form

Required if student has Diabetes

 

Over the Counter/Medication Consent-

Medical Update-Insurance Information Form

Required for all students grades 6-12

 

Due every "September" at the start of the school year

Prescription Medication Consent Form

Required if student might need other medication such as:  ADHD, Cystic Fibrosis, Midol, etc.

Seizure Action Plan/Medication Consent Form

Required if student might need Emergency Siezure Medication during school hours/activities

   

Immunization Record

Required for 11 year olds entering 6th grade

 

Tdap & Menactra

 

 

Physical Examination

Recommended for all students grades K-12

 

Required for students entering:

 

Kindergarten

Grade 5

Grade 7 or 8

Grade 9, 10, 11 or 12 

Sports Physical Examination “STATE” Form 

 

Required if student is playing sports

 

This is the only form used for athletes playing sports;
