Do you need a Sports Physical Examination from a doctor to practice or play with your team today?
You may go to Reconstructed Orthopedic Express at Total Turf, Lambs Road in Pitman to receive a complete Sports Physical Examination that also includes a cardiac clearance!
- Sports Physical Examinations must be completed on this "State" Form; no other form is acceptable for sports!
- You may obtain the form from the Middle School or High School office
Sports Physical:
- $35.00 - cash or credit, only!
- No appointment needed
- Walk-ins accepted
Hours of Operation:
- Monday - Friday 8:00 a.m. - 8:00 p.m.
- Saturday - 10:00 a.m. - 8:00 p.m.
*Injuries - ALL insurances accepted (excludes sports physical)!
