Allergens

 

We strive to keep all of our students safe while at school and allergies are taken very seriously.  Please take the time to review the following, if your child has a life threatenig allergy.

 

For use in the event of an anaphalctic reaction, please have your child's medial provider complete the order below (FARE Action Plan) for an Epinepherine auto-injector (EpiPen).

 

 

Please make sure that all of the required fieds are signed,

**including the parent/guardian field, which gives parental consent**.
