Medication Administration

 

  

 If your child needs medication (prescribed or over the counter) during the school day; please provide the signed and completed forms necessary as well as any medications ASAP. 

 

 

Medication or treatments must be in its original packaging, with the student's name visible.

**No loose pills or unlabeled medication will be accepted**

 

 The needed forms can be printed from this website (see the virtual nurse's office tab), or picked up from school during school hours.

 

All completed forms can be dropped off at the main office, emailed to [email protected], or sent in an envelope to Edison School (attn: Nurse)
