Absence/Tardy/Dismissal Reporting

Report absence/tardiness/dismissals before 8:45am everyday.  You may utilize the form found under Parent Resources on the Eisenhower website or by email to [email protected]. You must report absences every day your child is absent.

 

  • Include your child's full name, grade, homeroom teacher and reason for absence/tardy/dismissal.
  • Please specifiy the exact symptoms or illness your child is experiencing. 
  • If you child is ill on their Cohort Day, please also complete the student health survey in the parent portal in powerschool.
  • This is for current/future absences and tardiness only. For past absences send a note in with your child.
  • Your form submission or email verifies your child's absence, but does not necessarily excuse the absence. 
  • A seperate note in not necessary when you use the form or email the absence line. 
  • If your child goes to the doctor, always provide a doctor's note on return.
  • If an illness lasts longer than 3 days, a doctor's note is required to excuse the absence.
