Required 6th grade physical and immunization requirements

 

The New Jersey Deptartment of Education recommends that all students periodicall have a physical examination by their private physician for the protection of their health. The Bridgewater-Raritan Regional School District stongly recommends these examinations be performed on all students in the 6th and 10th grades.

 

Please be sure that all physical exams and immunization forms are returned directly to the Eisenhower Health Office before Sept 1st. 

 

If your child will not be 11 years old before Sept 1st, inform the Eisenhower Health Office of the date of your childs physical exam/immunization appointment. Students have two weeks after their 11th birthday to return the immunization record to the school. Please schedule appointments early. This will help ensure we are compliant with NJ State Law.

 

For those with no health insurance, The Somerset County Health Department offers FREE Vaccines through the Community Visiting Nurse Association. To schedule an appointment and find out if you are eligible please call: (908) 725-9355, EXT 0

 

For underinsured residents please call the Somerset County Department of Health at (908) 231-7155 for a listing of federally qualifies health centers that provide childhood immunizations
