Welcome to the Eisenhower Health Office
Although this year is starting out very different for all of us, I am looking forward to meeting you and having a great year with your child. Please feel free to email or call me with any question or concerns you may have, contact information below. To ensure your child’s health and safety, there are a few guidelines I would like to share with you.
Health Concerns: Please inform me of any changes in your child's health; any new injuries; recent surgeries; or new or changed medications.
Attendance: Period attendance is taken daily, even during remote learning. If your child is going to be absent please complete the absence reporting form found on the Eisenhower website under parent resources or email to [email protected]. You may also call in your child’s absence 908-231-0230, press 2 and follow the instructions. Please include your child’s name (spell last name if calling in); grade; homeroom teacher; and reason for absence. Any illness longer than 3 days requires a MD note to return to school.
Medications: If you child requires medication during school hours: medication administration forms are available online. Your doctor must fill out the appropriate forms and the parent/guardian must sign forms, and drop off the medication with completed forms. Prescription medication must be in the original container with the pharmacy label attached. Please contact me about setting up a date and time to drop off medication. Please do not come to drop off medication without setting up an appointment, visitation is restricted to the school at this time. Do not send medication to school with your child.
When students return to the building a health survey needs to be completed daily and submitted. We ask you to keep your child home with any temperature over 100 F; or any other COVID symptoms including: headache, sore throat, cough, fatigue, muscle aches, new lost of taste or smell, nausea/vomiting, or diarrhea.
If you are called that your child is ill from school, your child needs to be picked up immediately. Please have someone available to pick up your child if you are unavailable. Please update your contact information and emergency contacts so someone can be reached. If you child presents with any COVID symptoms, they will be directed to our isolation rooms located near the health office, while they wait for pick up.
Masks and Face Coverings: All students and staff are required to wear mask/face coverings while in school and on the bus. All requests for face mask exemptions must be submitted to the school nurse and reviewed by our school physician prior to your child attending in-person school. Below are the district mask guidelines:

Face masks must cover both the nose and mouth.

Face masks may not be shared.

Face masks may not have a vent.

Student's face masks should be labeled with first and last names.

Cloth masks must be washed daily in hot water.

Three-ply surgical type masks or two-layer cloth masks have been shown to be best in minimizing viral spread through the mask.

Face shields are not permitted as a replacement for a face mask.

Face masks, like other clothing worn at school, must be appropriate and adhere to the dress code.

Masks that are not acceptable: Vented masks, gaiter-type masks, bandanas, scarves, or a face shield without a mask.
