Dear 6th Grade Parents, 

 

Please be advised that The New Jersey Department of Health requires that all 11-year-old students entering 6th grade receive the TDap and Menactra vaccines.  Please send in documentation of your child's vaccines so that we can keep our records accurate and remain in compliance with the NJDOH . If your child is not yet 11, proof of vaccination within two weeks of their eleventh birthday is required. Thank you for your help in this matter and as always, please reach out if you have any questions.

**In accordance with the law set forth by the NJDOH, any student not in compliance with 6th grade vaccines will be excluded from school until vaccines are obtained and documentation is provided to the school.**
