2021/2022 School Year

Welcome to Mrs. Martin's 2021/2022 Health Office webpage.

Please feel free to contact me by telephone or email with any concerns regarding your child.

Check back for updates throughout the year!

Please do not scan/email any school forms unless requested to do so by the Nurse

HARD COPIES are required for:

Sports Physicals

Vaccine Records (including COVID)

Health History Updates

Medication Orders and Consent Forms

Asthma Action Plans/EpiPen Orders/FARE Allergy forms

All forms can be dropped off at the main office at any time, or mailed to my attention at RMS.

You can find printable forms on the menu to the left under the corresponding  heading

Sports Physicals are required for all students wishing to participate in RMS sports and activities.  They must be completed on the forms provided and signed by a parent/guardian and completed by a healthcare provider. They are valid for ONE YEAR FROM THE DATE OF EXAMINATION, not from the time the physical is recieved in the nurse's office.  Once recieved, they are reviewed by our district physician, Dr. David Gealt.

Impact Testing is good for 2 years and is required for every student athlete.  It is a baseline test which is used as a comparison tool in the event of a sustained head injury to help determine level of injury.

COVID VACCINES FOR 12-15 Year Old Students:

The FDA has approved the Pfizer vaccine for emergency use in 12-15 yr olds.  If you would like to have your child vaccinated, you can make an appointment at the Burlington County Mega Site at the Moorestown Mall, or check with your local pharmacy for availability of the Pfizer vaccine.  *please check with your pediatrician for any contraindications that may pertain to your child receiving the vaccine*

NJDOH is NOT requiring students to get vaccinated at this time. This information is provided as a courtesy for those who choose to vaccinate thier child and in no way is meant to influence that personal decision.

If you choose to vaccinate your child, and you would like to send in proof of vaccine, please do so by mailing or bringing in a copy of the vaccine card and your child's records wil be updated.  Vaccine status is taken into consideration when determining quarantine time as directed by Camden County DOH.

Thank you!

Please see district policies for COVID prescreening to be completed each day prior to entry in the building.  You can find the daily symptom checker at:
