**PLEASE HAVE YOUR CHILD VISIT THE NURSE'S OFFICE TO CHECK ON THEIR PHYSICAL/IMPACT/FAMILY ID STATUS IF THEY WOULD LIKE TO PARTICIPATE IN SPRING SPORTS- THEY WILL NOT BE CLEARED TO PLAY WITHOUT THESE**

  

 

 

 

If your child would like to participate in any sport/club in the spring, they will need a current sports physical on file.  Please have your child sign up in the nurse's office for a free physical .  If your child has had a sports physical within the last 3 months, they will need the Health History Update form filled out only.  This form is available in my office as well.  If your child does not recieve a physical at school, they will need to have the sports physical paperwork filled out by your physician prior to the start of practice. Impact tests are also required and are good for 2 years. 

 

** If your child has ever had a cardiac test such as Echocardiogram, EKG, or has ever been followed for a cardiac issue of any kind, please attach the cardiac clearance signed by your phsyician to your sports physical paperwork so that they can be cleared by our school physician **

 

 

Each athlete is required to register for Family ID. Please see the attached flyer with directions for signing up
