Dear Parent/Guardian,

 

In order to prepare for the upcoming school year, we are asking you to kindly review this letter carefully and fill out the appropriate forms.

 

All students entering the 6th grade are required to have both the Tdap and Meningococcal vaccines prior to starting school. Please send in the completed physical and updated immunization record to Eisenhower School prior to the first day of school.  You or the doctor may fax the forms to Eisenhower School at 908-231-1079, you may attach them in an email to me at [email protected] or you may turn them in at Eisenhower School.  The health office is open from 8:00 am – 3:00 pm, Aug 21-25, 2023.  The forms may be found on the Eisenhower website and are  attached below. A print out of your child immunizations is preferable.

 

For any students requiring any prescription medication at school: Medication needs to be in its original container with a prescription label attached. Medication forms need to be completed by both the parent/guardian and physician. All completed forms and medication need to be brought to the  Nurses’ Office on or before the first day of school by parent or guardian. Make sure to keep a copy of your paperwork for your records.
