Minimum Immunization Requirements for School

 

For Student entering Pre-Kindergarten:

 

Dtap (Diphtheria, tetanus, acellular Pertussis) - 4 Doses

MMR (Measles, Mumps, Rubella) - 1 Dose given on or after 1st birthday

HIB (Haemophilus influenzae type b) - 1 Dose given on or after 1st birthday

Polio - 3 Doses

Pneumococcal - 1 dose given on or after 1st birthday

Varicella - 1 dose given on or after 1st birthday

Influenza (Flu Shot) - 1 dose given during current flu season (due no later than 12/31 of current year)

Copy of most recent physical (within last 12 months) 

 

 

For Students entering Kindergarten:

 

Dtap (Diphtheria, tetanus, acellualar Pertussis) - 1 dose on or after 4th birthday or any 5 doses

MMR - 2 doses (1st dose on or after 1st birthday)

Polio - 1 Dose on or after 4th birthday or any 4 doses

Hepatitis B - 3 doses

Varicella - 1 dose

Copy of Most recent physical (within last 12 months)

 

 

For Students entering 6th Grade

 

 

Tdap (Tetanus diphtheria, acellular Pertussis) - 1 dose

Meningococcal - 1 dose
