Student ID/Lanyard Replacement

 

If your child is missing their ID please fill out the google form below. IDs are printed once a week, so your child will not immediately receive the new ID. They will be called to the office once it is ready. You can send the money for the new ID in a labeled envelope that I will keep locked up until the office lets me know that the new ID is ready!

 

The cost for a new item is as follows:

$5 – new ID card
$2 – new lanyard
$7 – new ID card and lanyard

 

If your child's ID is broken, there is no cost, we ask that you send in the broken lanyard so that we can send it to the office for a replacement.
