IF YOUR CHILD WISHES TO PARTICIPATE IN ANY OF THE SOMERDALE PARK SCHOOL ATHLETIC TEAMS OR THE INTERDISTRICT ATHLETICS OFFERED TO SOMERDALE PARK SCHOOL STUDENTS - THEY MUST MEET THE ATHLETIC PHYSICAL & HEALTH-RELATED REQUIREMENTS FOR APPROVAL

 Parents/Guardians & Athletes,

 Please be sure to read the “Athletic Participation Policy Parent Letter”  which outlines ALL the requirements necessary to be approved to participate on a school athletic team, squad, or intramural sport. Please pay close attention to Physical due dates as they are strictly adhered to in order to have sufficient time to process physicals prior to 1^st date of practices/tryouts.

 

  In addition per NJ State Statute, you must also review the (4) Flyers contained in the packet & sign the attached “Policies for Athletic Participation Parent/Guardian Acknowledgment Form” and return to the school nurse  in order for your athlete’s physical to be fully approved for participation on a team for practice, tryouts & games.

 

•  PLEASE COMPLETE THE REQUIRED FORMS & SUBMIT THEM TO THE SCHOOL NURSE NO LATER THAN THE DEADLINE INDICATED FOR THE SPECIFIC SPORT OF INTEREST
