Welcome Back Parents and Students!
Please take a moment to review this letter from the Health Office. Here you will find important information regarding NJ State and district requirements.
- If you have not already done so, please report to the School Nurse immediately if your child has any health condition, particularly those requiring medication of any kind (ex. an Epi-Pen or inhaler).
- If your child has Asthma or an allergy requiring emergency medication, an Asthma Action Plan or Allergy Action Plan is REQUIRED and must be updated YEARLY. Please also send in emergency medication immediately if you have not done so. This includes inhalers for students with asthma.
- Medications can be given in school by the Nurse, only if the Medication Authorization Form is completed by a physician or a physician’s order is received. This form is available on the Nurse’s website listed below. (This includes all OTC medicines)
- Immunizations must be up to date. Students entering Kindergarten and 6th grade are usually due for updated immunizations. You will be contacted by phone or letter if your student is not up to date.

- If your child is new to our school, a physical that has been completed within the last 365 days is required to be submitted 10 days from enrollment. If the physical is not submitted, your child will be excluded from school. Please contact the School Nurse immediately if you have any questions or concerns regarding this NJ State regulation.
- It is a NJ State mandate that every PreK student receive a FLU SHOT by November 30, 2026. If your PreK student does not receive a flu shot by that date, your child will be excluded from school until it is completed. This serves as a first notification of this state mandate. I will send reminders. *Please do not wait until the last minute! There are no exceptions to this mandate.*
- Lice L It happens. It is the PARENT’S responsibility to examine your child’s hair and scalp regularly for lice. Group lice checks are no longer a common practice in schools. Lice will be handled on a case-by-case basis, maintaining privacy. DO NOT send your child to school if you find lice. They must be treated for the lice before returning to school. You will have to ensure all eggs are removed to prevent reinfection. It can be a frustrating process, but be patient and vigilant.
- Screenings for height, weight, blood pressure, vision, hearing, and basic dental hygiene are conducted every school year for each student PreK-3rd grades. Beyond grade 3, vision is assessed in 4th and 6th grades only. Hearing is not assessed beyond 3rd grade, unless referred by a staff member or parent. Please notify the School Nurse in writing if you do not wish for your child to be screened.
- Ensuring proper dental care is the parent’s responsibility. Please ensure children are brushing at least twice daily and receiving regular dental exams. Our yearly dental assembly will take place in February during National Dental Month.
- If you have any questions or concerns, please contact me via phone or email.
*If your child has asthma, please submit an Asthma Action Plan my office after completed by a pediatrician. Please notify me immediately if your child has any health concerns.
*Please read EVERYTHING, as everything we post or send home is important. You can find many important forms and lots of information here. Never hesitate to call and ask if you have a question.
*Always send students to school dressed appropriately for the weather.*
Sincerely,
Tiffany Bohn RN, BSN, CSN, LNC, NC-BC
