Lawrence Township Health Department.

Free vaccines are also available through the NJ Vaccine for Kids programs.

Currently, children who are eligible to receive vaccines from the NJVFC Program must be within one of the following categories:
- Medicaid, Medicaid Managed Care, or NJ FamilyCare Plan A
- NJ FamilyCare Plans B, C and D
- American Indian or Alaskan Native
- Have no health insurance
- *Have health insurance that does not pay for vaccine
- Underinsured: A child who has commercial (private) health insurance but the coverage does not include vaccines, a child whose insurance covers only selected vaccines (VFC-eligible for non-covered vaccines only), or a child whose insurance caps vaccine coverage at a certain amount. These children must be immunized at a Federally Qualified Health Center (FQHC) or rural health center to receive free NJVFC vaccine.

Applying for determination of eligibility For children with developmental disabilities:
As the single access point into the
New Jersey Children's System of Care,
applications for eligibility for developmental disability services for children under age 18 must be submitted to PerformCare.
The application materials may be downloaded and printed out for completion by hand. You may also call PerformCare at 1-877-652-7624 to request that the application materials be mailed to you. Please refer to the instructions for guidance on completing the required application forms and submitting the required supporting documentation.
