LPPS Registration Requirements

 

**Registration must be completed online**


1. Children must be 5 years old on or before September 30th to enter kindergarten.

2. Only a parent or guardian may register a student.  Picture identification is required.

3. Registration for your child requires:

 a.  Social Security Card (card or copy)
 b.  Certified Birth Certificate (original)
 c.  Completed Immunization Record
 d.  Three (3) Current Proofs of Residence (CURRENT gas, electric, or water bill with parent/guardian name and SERVICE LOCATION listed or lease/cash sale in the parent/guardian's name)

4. These items MUST be uploaded to complete registration.
