Attention Parents and Guardians:
Medication administration, allergy emergency action plan and asthma action plan forms for 2023-2024 are on the school website. If you need a copy, please let me know.
Warren County Public Health Immunization Clinic
Warren County Health Department Vaccine clinic is open for children with NJ family care, Medicaid and no-insurance! To schedule an appointment or if you have any questions call 908-475-7960.

Reminder from Health and Attendance Office
IMPORTANT REMINDER! When calling your child absent from school, please state the reason for absence. If due to illness, please state the symptoms. The school nurse will follow up with any unreported absences as well as clarification of reason for absence if needed. Thank you for your understanding and cooperation!
6TH GRADE IMMUNIZATION REQUIREMENTS
The state of New Jersey has mandated that all students born after January 1, 1997 and entering grade 6 will be required to receive a booster dose of Diphtheria, Tetanus, and Pertussis vaccine (Tdap), as well the Meningococcal vaccine.
Students must provide documentation of these two immunizations from their Primary Care Provider. Students may not enter grade 6, without proof of the above immunizations. This applies to both in person and remote students.
For children without health insurance, these immunizations may be obtained through Warren County Health Department. Contact one of the public health nurses at 908-475-7960.
HAND WASHING - PREVENTS INFECTION
Hand washing: Before meals, after bathroom, after coughing-sneezing, blowing nose, before bedtime, and after coming in from outside play.
