6th Grade and New Transfer Students: The Clinton Township Board of Education requires a physical examination for all sixth grade and transfer students. The exam must be dated within one year of entry into school and needs to be documented on the NJ State Annual Physical Exam Form. A current immunization record should be submitted with the physical exam. Forms are available on this website.

If you do not have health insurance please contact us for alternate arrangements. We have also provided a link to NJ FamilyCare Health Insurance to assist you, if needed.

6th Grade Physical Forms are due in the Health Office by August 1st

6th GRADE STUDENTS CANNOT START SCHOOL UNTIL PHYSICALS & IMMUNIZATIONS HAVE BEEN SUBMITTED

Illness/Injury: A certified school nurse will assist with illness, injuries, or health related issues. In the case of serious injury or illness, the parents/guardians will be notified.  Please update CTMS if emergency telephone numbers change during the school year.

Please keep your child home if they have had a fever of 100.0 degrees or higher. The student may return to school when fever-free for at least 24 hours without the use of fever reducing agents. If the student has a sore throat, an upset stomach, or a rash, parents are encouraged to keep the student home and contact the family doctor. If the parent/guardian suspects that their student has a communicable disease, the student should be taken to his/her family doctor for diagnosis and treatment. Parents/guardians shall be required to contact the school and report any absence, and in particular report if the student is absent with a communicable disease. Please use good judgement before sending your student to school.

Notes from Physicians:  All letters from physicians, should be brought to health office initially. Teachers and coaches will be notified accordingly by the health office. Please be sure to submit a return to activities letter when your child’s condition resolves so it does not hinder future eligibility for athletics, physical education, field trips and/or activities.

Medications: State law does not permit students to carry prescription or over the counter medication while at school except in the case of potentially life threatening illness (Inhalers for Asthma and Auto-Injector Epinephrine for severe allergic reactions, and Insulin/Glucagon/Supplies for Diabetes).  If it is necessary for a student to receive medication during the school day, the school nurse will administer it.  State law requires a written prescription signed by your health care provider including diagnosis, medication dosage, route and time of administration, and written consent of the parent/ guardian. The medication is to be delivered to the health office by the parent in the original pharmacy container.  Forms are available in the health office and on this site. Medications other than inhalers, Epinephrine and diabetic supplies must be delivered to health office and picked up from health office by parents. 

All medications not picked up by the last day of school will be discarded.

Epinephrine, inhaler, diabetic and other required orders must be submitted in order for your child to participate in athletics, field trips and other school sponsored activities.

Screenings:   All students will be screened for Height, Weight, and Blood Pressure. In addition, 6th and 8th grade students will be screened for vision and 7th grade students will be screened for hearing and scoliosis.  A student may be exempt from the scoliosis exam if we receive written parental/guardian notification. Screenings may be requested by teachers, parents, or students if there is a health concern. A letter will be sent home for further medical follow-up on any screenings that fall outside of normal parameters as defined by NJ state code.
