Annual Health History Updates  (printed on green paper) were sent home in the back to school packet.  I have requested these forms that give me health updates, insurance information as requested by the state, and recent medical follow up.  It is extremely helpful for me to know your child's medical conditions and medications a student may be taking at home.  


If your child takes a medication at school you must have the appropriate medication administration form completed or asthma treatment plan completed by your doctor and signed by you, the parent or guardian.  No medication, including over the counter medications, may be carried by the student unless written medical authorization is on file in my office.  Please note that all medical forms are on my website for you to download as needed.


If your child has asthma or a history of anaphylaxis you should have the medication forms completed by the doctor or healthcare provider allowing your child to carry their inhaler or autoinjectable epinenephrine (epi pen).  Students must make sure they have their inhalers or epi pens at all school sponsored activities even field trips.


Sports physicals are held at the high school prior to every season, approximately 6-8 weeks before the season begins.  Students should listen to the announcements and see me for sign ups.  Physicals are held in May for fall sports since they usually begin in mid August. 


Students who have their personal doctor/healthcare provider complete their sport physical must hand paperwork in AT LEAST ONE WEEK prior to the start of practice to allow time for processing.  Our team doctor must review each physical.  All physicals must be on the state approved form that you can download from the FORMs section.  If your child carries an inhaler or epi pen, the medication form must be completed as well.
